What You Need to Know Moving Forward:
- Medical Records: Your medical privacy remains our top priority. All medical records will be securely stored by your medical provider for a specified period of time, and then securely destroyed. To request a copy of your records please request our Record Release Form before November 20, 2026.
- Final Appointments: We will continue to see patients for continuity of care through November 13, 2026. Please contact us promptly if you need a final refill or a referral.
- Inquiries: Our administrative team can be reached at 916-839-7740 until November 20, 2026.
