Please see your endocrinologist if you desire TRT replacement, and want to preserve fertility at the same time. If preserving fertility is a goal for you now or in the future, the right approach will be seeing a reproductive specialist before considering TRT. Brief discussion of your fertility preserving treatments will be offered to you at Wellness & Longevity during your initial consultation at no extra charge.How treatment works
- Provider exam, first visit ($100). Symptom intake, medical history, physical exam, and review of your lab results. The $100 fee covers the visit and the lab interpretation. Laboratory testing is a separate cost you arrange directly, which keeps your total down. You are welcome to bring relevant previous lab work you already have for comparison.
- Laboratory testing (you choose your lab). Bloodwork is required before any prescription, and patients must have indications and lab results confirming the clinical need for testosterone replacement. You obtain your labs through a low-cost direct-access lab such as JustLabs or DirectLabs, or a lab of your choice. We provide the lab order and review the results with you. Direct-access labs are usually far less expensive than insurance-billed testing.
- Lab review and treatment plan. We review your lab results together at the follow up visit. If the clinical picture supports replacement therapy, we prescribe and give FDA-approved testosterone cypionate injections.
- Weekly injections ($45 each). Injections are administered in the office, with limited doses available for self-administration while away from the office if needed. The skill of self-injecting will be taught to you free of charge. Any take-home doses are strictly monitored for any diversion in compliance with DEA and drug vendor guidance.
- Follow-up labs and visits. Recheck labs at three to four months after starting therapy, then annually. If therapy continues without gaps, no separate annual exam fee is charged.

What we prescribe
This practice prescribes FDA-approved testosterone cypionate injections only. We do not offer pellets. We do not offer compounded preparations. The reason is straightforward: FDA-approved testosterone cypionate has decades of clinical use, established dosing data, and a manufacturing standard that compounded products are not held to. For most men with confirmed low testosterone, it is the appropriate and effective choice.What ongoing care looks like
TRT is an ongoing medical relationship, not a one-time prescription. After starting therapy:- Initial follow-up labs at three to four months to assess response and adjust dose if needed
- Annual labs thereafter
- Visits to discuss how you are feeling, side effects if any, and whether the current dose still fits
- Monitoring for known TRT considerations including hematocrit, PSA where appropriate, and other clinical markers
Important considerations
TRT is not appropriate for every man. Reasons we may not prescribe include:- Active or untreated prostate cancer or breast cancer
- Severe untreated sleep apnea
- Uncontrolled congestive heart failure
- Elevated hematocrit or polycythemia
- Plans for fertility (see fertility note above; speak with an endocrinologist before TRT)
- Other medical conditions identified during evaluation
